What follows is info from a brochure (pdf file) we picked up at the Phoenix Earth Food Co-Op. I imagine it has been distributed to other health food stores. A "new program" begins in mid-September 2007. The Healthy Toledo Web site is still under construction in some areas.
I shortened a couple of the headings.

Changing the Health of the Glass City
"One life at a time"
How did I get here?
Have you ever looked at an old yearbook or wedding album and wondered, Did I really look that good? As the years pass by like freeway markers, responsibilities and stress surrounding your career, home life, and community involvement can squeeze out the priority to make healthy choices. The telltale bathroom scale tells a story ... a story many of us don't want to see or hear. You've gained ten, twenty, or forty more pounds. Now those extra pounds are challenging you to make a decision.
When you're twenty-five pounds too heavy, you're more likely to develop all kinds of chronic health issues. Heavier people are also putting more stress on their bones and organs. Did you that even a 10% weight loss can significantly boost your health?
America's weight problem is rapidly overtaking cigarette smoking as the leading cause of preventable death, according to the Centers for Disease Control.
On top of that, as a Toledo resident, you should be aware that Men's Health magazine recently ranked your city 98th out of the 100th Best and Worst Cities (for men) after crunching numbers in twenty-four categories, including frequency of exercise and life-and-death data on cancer, heart disease, and strokes. Toledo also has the highest smoking rate in the country, according to the Centers for Disease Control.
The Healthy Toledo campaign can help turn those daunting numbers around. If you're looking to change your diet and your life, then welcome to Healthy Toledo.
What is Healthy Toledo?
Healthy Toledo is a year long campaign that launches in September 2007. Participants can enroll at any time starting in mid-August 2007.
A new way of life
Healthy Toledo is about creating a new lifestyle. This program, which takes the slow, steady route to weight management and good health, means that you won't reach your perfect weight in a fortnight. This approach is more in line with the etymology of the word diet, which originated from the Greek word diaita, meaning "life, lifestyle, way of living."
This sixteen-week, four-phased program involves a comprehensive healthy eating plan, taking nutritional supplements, reserving time for and engaging in exercise, drinking plenty of water, and eating healthy snacks. Although Healthy Toledo comes with no guarantees, we're confident that you'll experience a personal transformation as well as weight reduction, decrease in poor health, and improved mental acuity and physical activity.
Don't go it alone
Healthy Toledo is about community. Before taking on the Healthy Toledo challenge, communicate your intentions to your primary care physician, friends, co-workers, family members and all those who can inspire you to move forward. Tell them that you'll need their encouragement as well as their accountability during this endeavor.
Plug in and spread the word
Healthy Toledo is about taking action. Once you've decided to change our diet and change your life, plug into the resources behind Healthy Toledo and feel better than you have in years.
Then invite your friends, neighbors, and co-workers to join you in this journey and help transform the health of Toledo.

History behind Healthy Toledo
The "Healthy Toledo" campaign began as a shared vision between Tony Scott, senior pastor of Cathedral of Praise in Maumee, Ohio, and Jordan Rubin, a best-selling and highly respected speaker on health issues.
Healthy Toledo, which ran as a pilot program for twelve weeks in the spring of 2007, was an unqualified success. The participants lost an average of 10 pounds*, decreased their body fat and Body Mass Index and improved their overall health.
Following the triumphs of the Healthy Toledo pilot program, the decision was made to take this program to the greater metropolitan area of Toledo. We are excited about citywide collaboration as well as key partnerships developing in the government, corporate, academic, medical, retail, and religious communities.
Healthy Toledo has become a community movement and collaborative campaign with a mission to transform the health of Toledo and its neighboring cities, townships, and communities.
This campaign will serve as the blueprint for a nationwide effort called Perfect Weight America, launching nationwide in January 2008 with the release of Jordan Rubin's new book, Perfect Weight America.
With Healthy Weight Toledo, the Glass City has an opportunity to lead the way and show Americans how they can improve their health and wellness.
* Participants lost an average of 10 lbs. in the first 10 weeks. (12 week data not reported at time of print)
